This webinar will feature a panel discussion between leaders from health insurance plans and state public health agencies highlighting integrated efforts to address components of Million Hearts in their communities. Panelists will discuss resources needed to address hypertension management; measurement and data collection mechanisms; efforts to address health equity; opportunities for future collaboration as well as sustainability facilitators. The panel will share challenges to these efforts and recommendations to help other states implement similar initiatives.Leaders from the Minnesota State Health Department and HealthPartners, and the New York State Health Department and EmblemHealth, will describe their experiences around hypertension control and Million Hearts and engage in a panel discussion.
